Riverside MyChart Terms and Conditions of Use

Riverside is pleased to offer you Riverside MyChart as a customer (patient) of Riverside and its affiliates. Riverside MyChart is an online service that provides you with convenient access to limited portions of your medical record. It also offers online communication with your physician and practice staff for prescription renewals, direct appointment scheduling, online messaging, and other web-based features. It is important to know how we manage information via the Internet. This Terms & Conditions Statement outlines our practices and sensitivity to your right to privacy. We reserve the right and discretion to revoke access to this service at any time. If you do not have access to features based on your type of activation or username and password, the terms and conditions for those features will not apply to you.

Please read the terms and conditions entirely and carefully before using Riverside MyChart. By accessing or using Riverside MyChart, you agree to be bound by all the Terms and Conditions of Use. Riverside reserves the right to change the features of Riverside MyChart and these Terms and Conditions of Use at any time without prior written notice. It is important that you review these Terms and Conditions of Use periodically as we may modify these from time to time, and you agree that such modifications are effective and binding upon you immediately upon posting of the modified version. The current Terms and Conditions of Use can be viewed at any time by clicking on the Terms and Conditions of Use link on this Riverside MyChart website.

Riverside MyChart does not contain your complete medical record. It is a summary of your record. If you would like to receive your complete record please submit a written request to your physician practice. Please allow [insert turnaround time here]. In addition, Riverside MyChart may not contain information from all your providers, including providers not affiliated with Riverside.

Website Availability

We try our best to make sure that Riverside MyChart is always working properly but there may be times when the system is shut down due to system maintenance, backups, or failures. Access is provided on an “as is available” basis and you are not guaranteed access at any particular time. We are not to blame for any problems that may occur from being unable to see your medical information.

Electronic Communication

We will make a best effort to provide a timely response to electronic inquiries. In some cases, the staff that needs to respond to an electronic inquiry or other communication may not be immediately available. Riverside MyChart is to be used for non-urgent messaging only. Please allow at least two (2) business days for a response. We are only able to respond to patient initiated communications based on the information provided by the patient. If additional information is needed for clarity, we may request additional information before responding to your request.

Emergency situations or conditions needing immediate attention should not be submitted electronically. If you have an emergency or require immediate medical attention, please call 911.

Email Privacy

Patients who are users of Riverside MyChart should be aware that they will be notified, if they desire, via email when there is new information to be viewed on Riverside MyChart. This means that any person with access to a patient's email will be able to see this notification. This could include the patient's spouse, employer, or anyone else that can access a patient's email account. Thus, it is the patient’s responsibility to protect the privacy of their email account.

Message Frequency

If you choose to receive notifications from Riverside MyChart, you will receive one (1) message per account update. Text notifications will direct you to your Riverside MyChart account for detail. Riverside MyChart also provides notifications to the patient email account. You will receive messages based on the preferences you select.

How to opt-out of our service
To Opt-out of notifications, you may go to the Preferences section within Riverside MyChart and modify your settings.

Text Notifications

Riverside MyChart users who select “text” as a preference, will receive notifications to the cell phone number provided.

Supported Carriers
Riverside MyChart users will receive a notification directly to their phone via SMS message, more commonly referred to as a text message from the following carriers:Alltel, Appalachian Wireless, AT&T, Bluegrass Cellular, Boost Mobile, Cellcom, Cellular South, Centennial Wireless, Cincinnati Bell, GCI, Immix Wireless, Inland Cellular, IV Cellular, Nex-Tech Wireless, nTelos, Sprint PCS, T-Mobile, U.S. Cellular, United Wireless, Verizon Wireless, Virgin Mobile USA, and West Central Wireless.

Cost
Riverside does not charge Riverside MyChart users for text notifications. However, Message and Data rates may apply from your individual cell phone carrier.

Minor Eligibility

An individual must be age 13 or older to request a personal Riverside MyChart account. Parents or guardians of minor children 0-12 yrs. of age may request access to the minor child's account while at any Riverside facility.

Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 13–17 may not view without consent of the minor patient. Because of these requirements, once a minor patient reaches the age of 13, parental access will be automatically terminated. If a parent wishes to receive access to a minor patient’s Riverside MyChart account after they reach age 13, please refer to the Proxy Access section below.

Proxy Access

Proxy access allows a parent, guardian, family member, or other trusted individual to view all medical information contained in a patient’s MyChart account. Proxy Access may be requested by submitting a Request For Proxy Access form to any Riverside physician practice. The form must be completed in full by both the Patient (or authorized patient representative) and the Proxy. If indicated, form submission should be accompanied by a copy of legal paperwork verifying the patient’s medical power of attorney designation.

● A proxy will have access to all MyChart information as if they were you.
● Proxy access covers all Riverside facilities and cannot be limited to one practice.
● Any trusted individual can be a proxy, such as a parent, spouse, adult child, or a caregiver.
● A proxy must be an adult (18 or over).
● A proxy is valid until revoked by the patient or Riverside
● Revocation will not apply to information that has already been released prior to revocation
● Authorizing the disclosure of health information is voluntary and that any disclosure of information carries with it the potential for an unauthorized re-disclosure and the information may not be protected by federal confidentiality rules.

Proxy Access for Teens

This section applies to all individuals 13 of age and over (Teens), who want to give permission to others to access their Riverside MyChart account. Teens give another individual access to their Riverside MyChart account by completing the Proxy Access portion of the Riverside MyChart Proxy Consent Form and submitting it in person to a Riverside facility. The individual selected by the Teen to have access to their account is called a Proxy. All medical information that is made available in Riverside MyChart will also be made available to your Proxy. Both you and your proxy will receive separate login identifications. Any patient age 13 and over may revoke proxy access in accordance with Riverside policy. Riverside reserves the right to revoke proxy access at any time for any reason.

Security and Confidentiality

We afford the same degree of confidentiality to medical information stored on Riverside MyChart as is given to medical information stored by Riverside in any other medium. We limit Riverside employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

For other than general information viewing, Riverside MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape or Internet Explorer versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the webpage comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Riverside by viewing the digital certificate. This certificate verifies the connection between the Riverside server's public key and the server's identification.

Userid and Password

User ids and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a Riverside MyChart user, your responsibility in maintaining the security of your medical information is:

1)     Changing your password on a regular basis, and
2)     Keeping your login ID and password confidential.

Riverside cannot be held responsible for:

● Unauthorized disclosure resulting from a user not logging out of an active session;
● Unauthorized disclosure resulting from a lost or stolen User ID and password;
● Unauthorized disclosure resulting from sharing a User ID and password;
● Unauthorized disclosure resulting from information printed by the user from Riverside MyChart; or
● Unauthorized disclosure resulting from personal computer settings or installed software products that may compromise information security.
● Any other use, access or disclosure outside the terms of this agreement.

Use of Third Party Services and External Website Links

You may choose to use other services provided by third parties in conjunction with your use of Riverside MyChart. Such services may include, but are not limited to, translation services, location services, credit card vendors, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor Riverside have control over the contents of these services, and neither Epic nor Riverside accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information be used and/or stored by a third party. If you decide to access any such services in conjunction with your use of Riverside MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.

Riverside MyChart may offer links to related medical Web sites not managed by Riverside. These Web site link(s) are for a patient's informational purposes only. Riverside does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.

Account Deactivation

Riverside MyChart will be deactivated due to specific terms and conditions for use of the application at the discretion of the physician practice and Riverside. The following is a list of reasons which will result in deactivation. The list is not inclusive and each circumstance will be evaluated individually.

● No longer a patient for that practice/physician
● Automatic system deactivation will occur for a parent to view a child's record when the child reaches the age of 13.
● Automatic system deactivation will occur after a number of failed login attempts to access the secure portion of the Riverside MyChart system
● Inappropriate use of messaging or scheduling (ex: excessive, unnecessary, inappropriate content) Abuse of proxy access
● Pending legal issues with patient or direct family
● Poor response to training
● Inability to use features without constant assistance
● Any other reasons that Riverside deems necessary

You will be notified by regular mail in the event your account is deactivated.

MyChart is a registered trademark of Epic Systems Corporation. All content in MyChart, including, but not limited to, text, photographs, graphics, button icons, images, artwork, names, logos, trademarks, service marks and data (the “Content”), in any form including the compilation thereof, are protected by U.S. and international copyright law and conventions. The Content includes both Content owned or controlled by Riverside and Content owned or controlled by third parties and licensed to Riverside. Except as set forth below, direct or indirect reproduction of the Content, in whole or in part, by any means, is prohibited without the express written consent of Riverside.

Privacy Policy

Our user’s privacy is a Riverside priority. Riverside protects your privacy by the following:

● Riverside does not directly market to you any services unless you have either provided written consent or opted in by cell phone, text message, email, or Riverside MyChart preferences.
● Riverside does not distribute any personal information about you, including your phone number, name, billing information, or any other piece of identifying information, accept for treatment, payment, or healthcare operations.
